General Release of Liability

In consideration of being allowed to participate in any way in Hurlburt, LLC related events and activities, the undersigned acknowledges, appreciates, and agrees that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Hurlburt, LLC, Inc. their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

COVID-19 Release of Liability

An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID 19 is an extremely contagious disease that can lead to severe illness and death. By attending any Hurlburt, LLC event, you voluntarily assume all risks related to exposure to COVID-19.

Photography Consent

I irrevocably consent to and authorize use of my comments and/or photograph or likeness, thereof, for advertisement, public relations, publicity, or any other non-profit purpose(s) by Hurlburt, LLC. I agree to waive any present or future claim for compensation or consideration. I also waive my right to inspect or approve the finished product, including written copy that may be created.

Code of Conduct

Any and all participants, both parents/guardians and athletes, engaging in any programming with The Volleyball Institute are expected to abide by the following Code of Conduct during their time in said program:

• Treat all other participants, coaches, and spectators with the utmost respect and dignity at all times.

• If there are any issues that one feels need to be addressed, they will be communicated to The Volleyball Institute leadership via email following a 24-hour ‘cooling-off’ period.
